VENDOR QUESTIONNAIRE
AMEC
Black & McDonald believes the supply chain is a major factor in delivering best
value possible to our clients. As vendors are important links in our supply
chain, the building of strong relationships with our suppliers and
subcontractors is a focus of AMEC Black & McDonald. In this regard, we invite
interested suppliers and service providers to apply for inclusion on our
Approved Vendor List. This list, an element of our supply chain management
program, is maintained by our Procurement Department.
As AMEC Black
& McDonald is evaluating submissions based upon what our current and future
requirements may be, please keep in mind the following when completing the
questionnaire:

Vendors are screened for capability and performance in providing goods and
services to the Oil and Gas industry; and

All questions should be answered in order to allow for a comprehensive and
balanced review of your submission. If questions are not applicable to your
business, they should be identified as “N/A”. If the answer is none, please
state “None” or “0”;
Once the
completed questionnaire has been submitted, AMEC Black & McDonald will review it
and further contact will be made to prospective vendors by our staff. All
submitted responses will be kept in the strictest of confidence.
